Clutch advice, please

Hi, after a long time away I've finally got round to looking at my clutch problems. I "finished" my twelve year restoration two years ago, had a few satisfactory local runs but experienced total clutch failure on the first proper outing (with my wife) and embarrassingly had to be recovered on a trailer.
The engine and box are both 1275 Midget, from different cars, and fully rebuilt. The clutch cover is the one which came with the engine, which looked ok, the plate was an NOS and the linkage is modified mechanical as supplied by Declan. I can't remember which fork and thrust bearing I used, but it was in accordance with recommendations on here.
If I manage to post photos you will see the friction lining is completely worn away on the pressure plate side, with a deep groove in the pressure plate on the line of the driven plate rivets. The box did seem to disengage and engage ok, (but there was a rather loud squealing up the short steep hill from home in first!)
Is this a problem with the components, or have I done something wrong when I put it together?
