Late style heater controls - how do (should) they work?

I have a slidey lever type thing on the front of the heater, but the labels are worn off. I assume this directs the air to different places?
There is a switch on the lower front of the heater, this doesn't do appear to do anything. Should this activate the blower? What about different fan speeds?
There is a cable pull on the right side of the lower dash that is labelled 'pull heater'. Again this doesn't seem to do anything. Is this the air temp control, or should it just do hot/cold with no in between?
The blower fan appears to be always off, and the heater constantly puts out hot air regardless of the controls.
