Heater valve woes

upon changing the heater valve (late type) today everything was going fine, all tightened up & hot run the engine, so all i had to do was finally attach the pull cable
so with a light tug at the inner cable to take up any slack before tightening it up, i got covered in hot water as the metal pressing which attaches to the cast alloy body just came away
(this is a NEW valve)
phoned the supplier who admitted it had happened before & said they`d send a new one out 1st post.
but although i wasnt injured, it made me wonder what would have happened if it had gone while driving,
anyone else experienced this ?
