Yes, I have known that to happen too - although I was actually revving up a hill in a low gear at the time. Made one hell of a bang and a very small dint in the bonnet! I have also had a piece break off the fan pulley whilst on the road, which made an 'interesting' noise too!

Because cracking of the fan blades is a well known problem the blade mounting holes should be checked for signs of radial cracking on a regular basis even though the stiffener plate may be fitted.
