Completing a V55/5

Hi all

Appologies if this has been covered before, I couldn't find it in a search.

Can anyone tell me, or possibly send me a scan copy, how to fill in the V55/5 form to apply to get Mollies registration onto the DVLA.

I have the V55/5 and V765 which I know I have to send to the club to get it verified, but I'm lost in the form. The obvious boxes I'm ok with (colur, body type, engine no. etc) but they don't all make sense to me.

Also, When I've finished the resto, we plan to change the colour to the later morris burgundy, so what coulour should I put on the form - grey as in the photo, or red as we intend her to be?

BTW the last photo on the signature was when we put the bits together to move house, she's stripped down to bare shell again now.
